MEDx Upgrade License Request Form

Use this form only if you already own a MEDx license, and wish to upgrade your current MEDx license to the latest version. Just fill in all the fields below with your name, system, and license information. You will receive an immediate response (via e-mail) confirming our receipt of your information. We will then generate and send you an upgraded MEDx Permanent License key.
